I'm not too worried about collisions here - it only has to be unique
within a folder, and is never shared outside that folder.

So I think:
"{seconds}.M{microseconds}P{pid}Q{count}.{hostname}.eml"
...would be fine. Having microseconds {M} and a unique (per-session)
sequence number {Q} should be enough. The PID helps a tiny bit too.
To get a clash you'd have to write a message, restart the app (magically
receiving the exact same PID), and write another message within the same
microsecond. That's a risk I can accept.

The {hostname} part doesn't really help, but is traditional- I think
it's to support writing into networked maildirs from remote machines,
which we don't support anyway.

The .eml is a bit sucky, but helps with OS search integration and
shouldn't really cause any harm.
