Several of the letters showed the CEO's last names crossed out and written in their place, their first names, suggesting a personal touch to the president's demands to the Big Pharma heads.

書簡では4つの改革を命じた。具体的には①低所得者向けの公的医療保険「メディケイド」加入者に、他の先進国で提供されている最も低い価格で医薬品を提供する②今後販売される新薬もメディケイドとメディケア（高齢者向け公的医療保険）向けに他の先進国で提供している最も低い価格を適用させる③ほかの先進国との薬価交渉を強化し、増収分を米国の薬価引き下げに充てる④中間業者を排除し、消費者や企業に医薬品を直接販売する仕組みを導入する――ことを求めた。
60日後の9月29日までに命令に従わない場合、「私たちが持つすべての手段を動員する」とトランプ氏は警告した。
この発表を受け、書簡を受け取った製薬大手の米ブリストルマイヤーズスクイブ（BMS）の株価は前日（30日）終値に比べて一時、5%以上下げた。米メルクは4%以上、米ファイザーやイーライ・リリーもそれぞれ2%以上下落した。
米投資銀行ジェフリーズのアナリスト、デビッド・ウィンドリー氏によれば、トランプ政権の薬価引き下げ政策で大手製薬企業の売上高が2030年までに約700億ドル（約10兆円）減少する可能性があると指摘している。
トランプ氏は5月に薬価引き下げを求める大統領令に署名していた。だが、トランプ氏は31日に明らかにした製薬大手CEO向けの書簡で、「（製薬大手のこれまでの対応は）責任転嫁と補助金を求める内容ばかりだった」と不満をあらわにした。
