それとも文法的に間違いなのか？
大学受験の問題とかもそうだけど、
こういう文法問題で理屈なくそういう言い方はしないから間違い、

統語論上の問題ではなく、意味論上での問題だと思う。
Cambridge English Dictionary:
[truth] a fact or principle that is thought to be true by most people:
It would seem to be a general truth that nothing is as straightforward as it at first seems.
Oxford English Dictionary:
a statement or principle that is generally considered to be true:
ほとんどの人によって一般的に信じられていることが truth だとすると
みんながびっくりすることはないと思う。

意味的な問題に加えて、コロケーションとして truthという名詞に同格のthat節が続くことはほとんど（あるいはまったく？）ない、ということがあると思います。
Ngramで、"the fact that"と "the truth that" を比較すると、圧倒的に前者が多く、後者の用例は稀です。
また、その稀な the truth that の 使用例においても、そのthatのほとんどは関係代名詞のthatであるようです。

40例が表示され、そのうち同格の that と思われるものは9例でした。

truth が同格のthat節を伴うこともあるんですね。
私もSKELLで検索してみたところ、やはり40例が表示され、9例が同格のthatでした。
その9例の例文もほとんど一致していましたが、
It was not okay to accept the truth that we needed help.
が欠けていて、代わりに
They believe the truth that God sent Jesus.
という例が表示されました。
尚、the truth that の検索結果に、“0.89 hits per million”と表示されます。これはたぶん出現率を表しているのでしょうが、millionというのが何を数えてるのか分かりません。

million というのは、コーパスに含まれる「文」を数えてるようです。
SKELLのコーパスは5700万以上の「文」、10億以上の「語」から構築されているらしい。

>英語ネイティブに確認してみましたが、ご質問の文で"in what..."という表現を使うと、「国内初のケース」であるという事実を強調する効果が得られるのに対して、"which"を使うとたんなる「補足的な説明」ということになるそうです。
なお、この表現はニュースの記事だけでなく、日常的な会話でも多用するそうです。

どのように検索したら the truth that 同格の例文が見つかるか試行錯誤して見ると、
1つの例として "the truth that women" で検索すると、たくさん例文が見つかることが分かった。

"Rebracketing" (also known as "resegmentation" or "metanalysis") is a process in historical linguistics where a word originally derived from one set of morphemes is broken down or bracketed into a different set.

How farで距離を聞く場合は、答える側としては程度なので疑問文を平叙文に戻した場合は、aboutのような文内で副詞を表す表現がつくと考えても問題ないでしょうか。
(Howは平叙文内の形容詞、副詞を聞くため)
聞く側: How far can you swim?
答える側: It's about 5 kilometers.
また、最初の疑問文を平叙文にする場合、このような形になるでしょうか。
How far can you swim?
↓
You can swim about 5 kilometers.
聞く側としては、about 5 kilometers.がわからないため、副詞句部分をHow farで聞いている。
